Usually, the old doors are removed (although we've also built while leaving them installed to "cover-up clutter")
The storage cabinets above the desk extend on up into the upper recesses to the ceiling, but we built them so the doors would open (wasn't that nice of us!) below the door header.
The lower cabinet is in another house, built so that the owners would have a long working counter with plenty of adjustable storage space below.
